ACA Health
Forms
It's that time of year again — the decision about the many forms
a camp needs to have for their campers and staff. If those forms include
HEALTH FORMS, ACA can help!
While a camp does not have to use Health
Forms provided by ACA, many choose to
do so. The main thing to remember is that any health form used by an ACA-Accredited® camp
will need to ask the questions requested in the Standards (HW-2 and HW-6). Those
provided by ACA meet the Standards.
Used together for overnight camps, the
Camper Health History
Form 1 (four-page
form completed by parent/guardian) and the Camper Health
Care Recommendations Form 2 (one-page form completed by physician) replace
the old "Green form.” The
newer forms more specifically address many concerns of the camp director including
allergies, dietary restrictions, mental, emotional, and social health of the
camper, etc. Day camps can use the Camper Health History alone in order
to meet ACA Standards. These forms are available to ACA members/camps as
a free
download and
are NOT customizable.
The above two mentioned forms as well as additional health forms which can be
used by camps are available at: www.acacamps.org/protected/healthforms/ (password
protected site). These forms are also made available to ACA members as
a member benefit and can be downloaded from this site for no charge. Many of
the forms are customizable for your camp (you can place your camp logo and address
in specifically designated "boxes"). The customizable forms include
Staff Health History, Medical Recommendation for Camp Employees and other useful
health-related forms. Make sure to check them out!
The Camper Health
History Form and the
Camper Health Care
Recommendation Form,
along with a variety of other forms are also available for purchase in one hundred

Accreditation and the Statement of Compliance
By now, a legal representative of all ACA-Accredited® camps
should have signed and submitted their
2008 Statement of Compliance (SOC). Annual
submission of the SOC confirms their compliance
with applicable laws and codes and with
ACA standards and ethics throughout the
entire year and is a required part of the
accreditation process. Without the
current signed Statement of Compliance,
ACA has the authority to remove Accreditation.
Accreditation applies to all camp programs
and services offered under the same camp
name throughout the year, except single
day events.

Was Your Camp Visited in 2008?
If so, you should receive a letter from
your Section office with the official
notice of your accreditation status no
later than November 10. If
you have not received a letter by this
date, please contact your Section office
directly.
All camps re-accredited or newly accredited
the summer of 2008 will receive a new,
metal Camp Accreditation sign before the
end of the year.
All ACA-accredited camps will be receiving
their 2008 Accreditation Certificate in
late winter 2009.

Will Your Camp Be Visited in 2009?
It's not too early to start preparing
now! Below are a few ideas shared
with us from recently accredited camps:
- Make sure you have the most
current set of revisions! Accreditation
Process Guides purchased after February
2008 came with this supplement (hint:
this printing has black and white pictures).
- DIVIDE AND CONQUER! Divide
the sections by areas of responsibility.
Not only does this spread the workload,
it also involves other key staff members
in the educational process.
- Establish a schedule. Work
on two sections of the Standards in December,
two sections in January, etc.
- Check out the resources on the ACA
Web site: www.ACAcamps.org/accreditation.
- Attend a Standards Course!
- ASK QUESTIONS.
- Request a Written Documentation
