2013 Central Registry Clearance Update for Michigan Camp Licensing

Old Method – Take or send “Central Registry Clearance Request, DHS-194, or DHS-1929” to your local DHS office or send them to the DHS office in the county where the prospective employee resides.

New Method - beginning immediately, send to Kim Graza, Organizational Support Manager, BCAL, 7109 W. Saginaw Highway, Lansing, MI 48917.  The link to the new Central Registry Form.  NOTE: The form link directions are not up-to-date; disregard, “Mail or take the completed DHS-1929 to the DHS office in the county where the employer or volunteer agency is located.”
