A permit is required to operate an organized camp through the LA Department of Health and Hospitals as administered by the Parish Health Units.

An individual must fill out the Authorization Form and General Disclosure form. All forms available from the Louisiana State Police.
The cost is $31.00 (includes $5 technology fee). For FBI processing, where authorization is required, there is an additional $12.00 fee. Results are returned within 6–8 weeks.
